There are many reasons a drain may block.

This was not an exact science and in some instances there was a lot of digging and concrete cutting required trying to locate the issue. Nowadays we have the advantage of CCTV inspection cameras. These cameras insert into the drain and can exactly locate the source of the blockage.
Water jetting is a common way to clear blockages. The high pressure system basically blasts the blockage until it has cleared.
An electric eel can also be used to clear tough to move blockages. The Electric Eel works by a motor spinning a steel cable with a cutting head attached to the end. The cable is fed into the pipes and will break down the blockage once it comes into contact with it.
